Applying to join the UKCHIP Register
To be accepted onto the register you need to complete the On-line application form. This form requests information covering a number of areas:
- Personal details
- Qualifications
- Current employment
- Previous employment experience
- Professional memberships and registrations
- Previous professional development
- Continuing professional development (CPD)
- A supporting submission
- The names of a sponsor and a referee
- A declaration of adherence to the Code of Conduct and standards of UKCHIP
The UKCHIP application form can look daunting at first sight. However there are a number of reasons for this:
- UKCHIP assessment needs to be of sufficient detail or the value of registration is diminished
- There is no single route into health informatics so assessment is based on your qualifications, experience, previous and continuing development and any other relevent factors
- The form has been designed to allow applicants from different backgrounds the opportunity to demonstrate how they meet the requirements
Before you start to complete the form it is useful to have either a recent CV or job application form to hand. You will find you need dates and details of your qualifications and previous experience. Click here for a full list of what is required to complete the form.
Remember most of the free text boxes do not require a large amount of information, e.g. the responsibilites of your current post should be summed up in just a few sentences - you do not have to enter your full job description. You are not competing with other applicants, as in a job application form, just providing enough information for our assessors to decide whether you meet the criteria for the level you have applied for. If the assessors need more information we will ask you for it before they make a decision. See examples of completed application forms
See the menu on the left for more detailed guidance and help on the registration process.
If you have any queries about completing the form please contact the Registrar on registrar@ukchip.net.
Submitting & Processing your application
When you have completed all parts of your online application form, click the 'Submit' button.
You will be taken automatically to the secure online payment site. You must pay your registration fee online by credit / debit card or arrange payment with UKCHIP before your application can be processed.
Your sponsor will be contacted and asked to confirm their support for your application. Please ensure your sponsor is aware and available to respond to this request before you submit your form.
Your application is sent to one of the UKCHIP assessors to determine whether you meet the criteria of the level you have applied for. If the assessor declines your application or feels it is borderline, it will be referred to a second assessor. We may also contact your referee or give you the opportunity to provide more information.
We normally aim to process applications within 28 days, as long as your registration fee is paid and your sponsor has confirmed your application.
